Patient Information

We accept all private insurance plans and Medicaid HMOs, including Title 19, BadgerCare+ and Medicare. Patients should bring insurance information with them to their first appointment for accurate billing.

New Patients

We are currently accepting new patients. To schedule a medical or dental appointment at either of our clinics, please call (414) 935-8000. Same day appointments may be available depending on provider schedules, but we request that you call ahead.

If you are a new patient coming for your first visit, please arrive 20 minutes early to fill out paperwork and complete important forms. Also, please bring the following:

  • Insurance information (if you have insurance)
  • Immunization (shot) records
  • A list of any medication you are taking
  • Any medical records from your previous doctor

Benefits Assistance

A financial counselor is available to assist you with determining your eligibility and applying for Medicaid, FoodShare and childcare benefits. If you do not have health insurance, or your insurance does not cover all of your health costs, our financial counselors can help you apply for the sliding fee program (see details below).

Sliding Fee Program

To ensure that everyone has equal access to health care services, the sliding fee program helps people who have limited or no health insurance. Patients without insurance will be charged based on their income and the number of people in their household.

To apply for the sliding fee program, you must provide the following documentation:

  • Proof of income (one of the following)
    • Check stub
    • Letter from employer verifying wage
    • Copy of your most recent federal tax return
  • Identification (one of the following)
    • Driver’s license
    • State identification card

Payment Options

All co-payments are expected at the time of service. We accept cash, personal checks, and credit cards (Visa, MasterCard, Discover).
